The PPO $3500 plan with Anthem might refer to two (if not three) different plan options on the Individual health insurance market in California. Two of these plans are no longer available but many people still have these plans.
The first is viable and is probably one of
the top 2-3 options on the market for mid level
benefits. We'll look at the other two but also
delve a little deeper into the last remaining
$3500 Smart Sense plan.
Anthem brought out a PPO $3500 plan a few years ago and it was a hit. It was a straight $3500 deductible and max which made it one of the lowest out of pocket maximums on the market.
It did not cover office visits with a copay
(they were subject to the deductible) but
medications (both generic and brand) were both
available. The plan is no longer available and
it makes sense to check your benefits if you are
on this plan.
The Smart Sense $3500 is available and has been the plan of choice if you're not looking for straight catastrophic coverage in California. You get 3 copays right away at $30 in-network per family member for the calendar year.
You have a $3500 deductible and a $3500 max out of pocket. There are two options for medication: generic only or generic and brand name coverage (other benefits are the same). The deductible level has been the "sweet spot" in terms of benefits versus pricing.
There are lower and higher deductibles (even within the Smart Sense suite) but the $3500 has proven to be the best value for many people.
